Background of the Study
Maternal health insurance plays a crucial role in ensuring that pregnant women receive quality healthcare services, reducing the risks of maternal and infant mortality. In Nigeria, the National Health Insurance Scheme (NHIS) and other state-led programs aim to provide financial protection for pregnant women. However, enrollment in these programs remains low, particularly in rural and underserved areas such as Zamfara State (WHO, 2024).
Zamfara State has one of the highest maternal mortality rates in the country, largely due to inadequate healthcare access, financial barriers, and low levels of health insurance coverage. While maternal health insurance programs exist, many women are either unaware of them or face challenges in enrolling. Factors such as poor healthcare infrastructure, cultural beliefs, bureaucratic hurdles, and economic constraints hinder participation in these schemes (UNICEF, 2024).
This study seeks to identify the key barriers to enrolling in maternal health insurance programs in Zamfara State and propose strategies to improve access and utilization.
Statement of the Problem
Despite the availability of maternal health insurance programs, enrollment rates remain critically low in Zamfara State. Many pregnant women continue to rely on out-of-pocket payments, which often lead to delays in seeking medical care or the use of unskilled birth attendants (Guttmacher Institute, 2024). The reasons for this low enrollment include lack of awareness, cultural perceptions, administrative challenges, and economic hardship.
Additionally, many healthcare facilities do not fully implement NHIS services, leading to mistrust and discouraging women from enrolling. Without adequate enrollment, the benefits of maternal health insurance programs remain limited, and maternal and neonatal health outcomes continue to be poor. This study aims to explore these challenges and provide evidence-based recommendations for increasing enrollment in maternal health insurance programs in Zamfara State.

To identify socio-economic, cultural, and administrative barriers to maternal health insurance enrollment.
To recommend strategies for improving maternal health insurance enrollment and accessibility.

Scope and Limitations of the Study
This study will focus on pregnant women and new mothers in Zamfara State, evaluating their experiences with maternal health insurance enrollment. It will include perspectives from healthcare providers and policymakers. Limitations may include difficulties in obtaining official data on health insurance enrollment and variations in healthcare policies across different local government areas.
